Description Archibald's sign (positive metacarpal sign/metacarpal sign/metacarpal finding) is a descriptive term for shortening of the 4th and 5th digit presenting as dimpling over the knuckles of a clenched fist. This sign describes the difference in the relative lengths of the lateral three metacarpals and is determined in the radiograph of the hand. An extension of the line, drawn tangentially to the circumference of the distal ends of the 4th and 5th metacarpals normally passes distal to the head of the 3rd metacarpal (negative metacarpal finding). Sometimes this line is tangential to the circumference of the head of the 3rd metacarpal (borderline metacarpal finding). When the line runs through the distal end of the third metacarpal it has been termed as a positive metacarpal finding. A positive metacarpal sign can be seen in gonadal dysgenesis,pseudohypoparathyroidism of Albright's hereditary osteodystrophy, brachydactyly, rarely with homocystinuria and as a normal variant in about 2%-4% of the healthy subjects.
