Description Most inflammatory rheumatic diseases more often affect women. A significant proportion of these patients become ill at a young age and are therefore confronted with the issue of the influence of illness and therapy on fertility and the course of pregnancy. Due to the evidence of adverse effects of disease activity on fertility and pregnancy outcomes, the goal is to achieve the lowest possible activity of rheumatic disease in the preconception period and during pregnancy. It is necessary to choose safe therapy, that could optimally control the disease and doesn't endanger the successful fetal development. The management of this problem can't be done without the close cooperation of the rheumatologist, gynecologist, obstetrician and the patient itself. With the increasing therapeutic potential in rheumatic diseases, there is a growing chance to effectively treat these patients before conception and during pregnancy, and thus the chances of successful maternity for women with rheumatic diseases have increased significantly today. In the absence of evidence-based medicine, our therapeutic decision-making in pregnancy and lactation is based on case-reports and a lack of evidence of adverse effects. The following case study present the course of active axial spondyloarthritis in pregnancy and the possibility of therapeutic use of TNF inhibitors in a pregnant patient.
