Description Intensive care has a very specific environment. In a relatively limited place, there is a high concentration of patients with the failure of many organs, requiring a large number of staff and technical equipment. Many processes, communication trajectories and priorities accumulate. The patient's condition may change suddenly and significantly. In the situation of patient admission, we are also limited by information and time. In a short time, we must evaluate the patient and determine the priorities of the highest importance. Therefore, care processes must be set up so that we can respond smoothly and quickly to the changed state of the patient. A specific situation is the transport of a patient outside the ICU environment and his transfer to the care of another team.
