Description Background: Cor triatriatum sinistrum (CTS) is a rare congenital heart disease consisting of a fibromuscular membrane that subdivides the left atrium into two chambers. Most patients have onset of symptoms within the first year of life. Patients with isolated CTS and no restrictive orifice may be asymptomatic and accidentally diagnosed in adulthood. Depending upon the orifice size in membrane, the hemodynamics may mimic mitral stenosis. CTS limits the ability of the heart to respond to increased cardiovascular demands. Case report: In this report, we present a patient in whom the diagnosis of CTS was made during pregnancy. The woman developed signs of heart failure at the third trimester of gravidity. Despite CTS means high risk of maternal mortality and morbidity, the pregnancy was continued and Caesarean section was scheduled at term. The increase of intravascular volume in the early postpartum period caused acute heart failure with severe mitral regurgitation and pulmonary hypertension. The mechanism of mitral regurgitation is unclear and has not been documented in the literature yet. The patient required diuretic therapy and careful monitoring. Finally she was stabilized and discharged home. After 3-month follow-up there was a regression of mitral regurgitation and pulmonary hypertension disappeared. Conclusion: There are no recommendations for the care of pregnant patients with CTS and there are also small numbers of documented cases. We describe a case report which proves that the patient with CTS can deliver a healthy newborn.
