Description Current advanced analytical methods are very specific, and sensitive and make it possible to determine substances in very low concentrations in various biological matrices. For a long time now, the application of these methods for determining drug levels in a dried blood spot has appeared to be attractive. However, the practical feasibility is limited by many factors (low sample volume (30-50 µl), influence of the matrix (hematocrit, haemoglobin, erythrocytes)). Similarly, the unclear relationship between capillary blood and plasma concentration complicates interpretation. Methods: MS detector qTOF (Bruker Daltonics), and ESI+ ionization was used for analysis. An LC-MS method was developed and optimized for determining the antipsychotics clozapine, olanzapine, and risperidone. Results: A method was developed to extract clozapine and olanzapine from a dried blood spot using a deuterated internal standard. Analytes were separated on a C18 reverse-phase column with solid core particles. The method was applied to a pilot analysis of the pharmacokinetics of clozapine in the laboratory rat, and the correlation between the plasma concentration of olanzapine and the concentration of olanzapine in a dry blood spot was verified using this method. Discussion: This minimally invasive method makes it possible to determine the pharmacokinetic profile of clozapine. A correlation was found between plasma olanzapine concentration and capillary blood concentration determined from the amount of olanzapine in the dry blood spot. Based on the practical experience from these applications, important aspects of the practical use of the dry blood spot were evaluated.
