| Description | Frequent negative consequence of chemotherapy is ovarian damage and premature ovarian failure (POF) with subsequent infertility. Project primary aim was prospective case-control study of gonadotrophin-releasing hormone analogues (GnRHa) administration to patients with Hodgkin Lymphoma (HL) during chemotherapy to prevent ovarian damage depending upon chemotherapy regimen. Project secondary endpoint was development of ovarian tissue cryopreservation (OTC) as modern and potentially effective method of reproductive function protection. Study group consists of 72 patients in fertile age treated by chemotherapy together with GnRH-a administration. Number of women with POF after chemotherapy in study group was compared with control group of 45 fertile women without GnRH-a protection. Patients receiving GnRH-a during chemotherapy showed statistically significantly less cases of premature ovarian failure (POF) than control set of patients. Comparative analysis depending on cytotoxicity of chemotherapy regimen used showed significant reduction of POF risk in women treated with less aggressive chemotherapy regimens. Project further describes medical and scientific knowledge and authors own experience with OTC technique as novel and progressive method of ovarian function protection. Ovarian tissue was obtained during laparoscopic surgery and later cryopreserved in 5 nuliparous women with malignant lymphoma diagnosis before chemotherapy during years 2005_2007. The OTC represents effective alternative or addition to the cryopreservation of embryos or oocytes for women at risk of POF before planned chemotherapy. All methods of reproductive function protection require close multidiscipline cooperation among oncologist, reproductive medicine specialist and embryologist. |
