| Description | A recrospective analysis was carried out of a sample of patients hospitalised in the Clinic of Internal Medicine. Gariatric Medicine and General Medicine in the period 2005-2007 Acute coronary syndrome was diagnosed in 92 patients from the full sample and surgery was carried out in 41 cases (44,6%). In 51 cases (55,4%) a conservative approach was adopted or it was not possible to carry out coronary surgery. The most frequent reason for the selection of a conservative approach was acute ar chronic severe cognitive defects (delirium, advanced dementia), as well as severe polymorbidity or severe acute complications outside the cardiovascular system. Conclusions for practice. There are significant inter-individual differences for patients over 75 years of age in terms of acute coronary syndrome diagnosis. Patients over the age ov 75 usually come for treatment for acute coronary syndrome significantly later than younger patients. Patients over the age of 75 with previously diagnosed ischemic heart disease often fail to get treatment according to the current recommendations of expert societies. In this age group acute coronary syndrome is relatively frequently associated with an ongoing infection. In this care, the symptomatology is fundamentally modified. Surgery should always be taken into consideration as the most effective modality for patients over the age of 75 and age itself is not a contraindication form surgery. Mortality for acute coronary syndrome treated by coronary intervention surgery in the over 75 age group does not differ significantly from that of younger age groups. |
