Description Objectives: Improvements in coronary angioplasty techniques combined with increasing experience in individual facilities have been raising the age limit for patients with acute coronary syndrome who can potentially undergo coronary angioplasty with a reasonable benefit in terms of additional years of quality life. The aim of our retrospective study was to determine the current situation in the treatment of acute coronary syndrome for patients over 75 years of age in the Brno Faculty Hospital in co-operation with two of its departments. Design, settings, participants. A retrospective analysis was carried out of a sample of patients hospitalised at the Department of Internal Medicine, Geriatrics, and General Practice in the years 2005 through 2007 because of chest pain or suspected coronary syndrome. Measurements. Risk factors, previous medication, the method used to treat acute coronary syndrome, contraindications for invasive approach, changes in medication after the episode and overall outcome were evaluated. Results. Alltogether 126 patients were evaluated. The occurrence of risk factors was high - hypertension in 68%, dyslipidemia in 48% and diabetes mellitus in 38%. Acute coronary syndrome was diagnosed in 92 patients from the full sample and invasive approach was carried out in 41 cases (44.6 %), two thirds with optimal result, mortality of the procedure was comparable with younger patients. Resting 51 patients (55.4 %) were treated conservatively. Conclusions for practice. Surgery should always be taken into consideration as the most effective modality for patients over the age of 75, and age itself is not a contraindication for surgery. Mortality for acute coronary syndrome treated by coronary intervention surgery in the over-75 age group does not differ significantly from that of younger age groups. Patients over the age of 75 with acute coronary syndrome often fail to receive treatment according to the current guidelines.
