Description Objective: The authors evaluated the results of visual function after implantation of 3 types of diffractive multifocal lenses (MIOL): AcrySof ReSTOR +4.0 D (Alcon), AcrySof ReSTOR Aspheric +4.0 D (Alcon) and Acri.LISA Aspheric 366D (Carl Zeiss Meditec & Arci.Tec). Methods: The group 71 eyes of 37 patients after cataract surgery were divided into 3 groups according to type of implant: 1st group (AcrySof ReSTOR SN60D3, n = 21), 2 group (AcrySof ReSTOR SN6AD3, n = 23) and 3 group (AcriLisa 366D, n = 27). The mean age was 57.3 SD 4.9 years and follow-up 13.75 SD 7.5 month of preoperative spherical refraction +1.47 SD 2.43 D and preoperative uncorrected visual acuity for distance (UCVA) 0.29 SD 0.17. The authors compared the UCVA for distance (ETDRS optotypes) in the near and intermediate vision monocular (Jaeger table, the distance of 30 and 50 cm), refraction, contrast sensitivity (CS) for mesopic conditions in 4 spatial frequencies (CSV-1000E board, VectorVision) and higher order aberrations and spherical aberration HOA SA (aberrometer Zywave, Bausch & Lomb, 6 mm pupil size) 3, 6 and 12 month postoperatively between the three groups identified above. Results: The mean postoperative UCVA to the distance moved in all third treatment groups in the values of 0.77 SD 0.2, 0.2 and 0.78 SD 0.78 SD 0.1 to postoperative refraction for distance -0.7 SD 0.4 D, 0.3 SD 0 4 D and +0.03 SD 0.6 D. Near vision to achieve the best results in the 1st group (average J No. 1 without addition), 2nd and 3. group about J # 1, 7 by addition of +0.57 and +0.8 D. The difficulty with vision in the middle distance were recorded in 1 and 2 group with the necessity of the addition -0.85 D and -0.5 D in KC mesopic conditions in the 1st group was below the lower limit of normal at high spatial frequencies (12 and 18 c / deg) throughout the study (2.3). In 2 and 3 reduction in group KC was diagnosed at higher spatial frequencies only 3 month after surgery (3.8), from 6 KC month reached preoperative levels. Mean square error (RMS) for higher order aberrations and spherical aberration HOA SA was lower in the 2 and 3 group - aspheric implants Conclusion: The types of diffractive multifocal implants provide good distance vision and near without glasses with aspheric correction and adjustment by reducing the negative perception of optical phenomena (increased contrast sensitivity and reduce the incidence of higher-order aberrations, respectively. spherical aberration).
