Description In recent years, the measurement of hair cortisol has emerged as a tool for long-term, individualized monitoring of therapy in psychiatric, endocrinological, and oncological patients. However, cortisol levels in hair decrease from the roots to the tips due to natural washing-out processes. This decline can be classified as exponential, and by employing an appropriate probabilistic model, it is possible to quantify the initial cortisol concentrations that would have been present in the zeroth segment historically. These concentrations would provide novel insights that would otherwise be unavailable without additional hair sampling. This paper introduces two independent approaches for the probabilistic quantification of hair cortisol decline. First, a hierarchical model utilizing 6 cm segments from 37 volunteers, and second, repeated sampling of 10 one-centimeter segments from a single volunteer. Accurate determination of the initial concentration (zeroth segment) will enable significantly improved control of therapy and monitoring of episodes and interventions in patients.
