Endoscopic Submucosal Dissection in a Low-Incidence Gastric Cancer Population: Outcomes from a Tertiary Center in the Czech Republic

Description | Aims Gastric cancer remains a signifi cant disease, although its incidence is steadily declining. Endoscopic techniques, particularly endoscopic submucosal dissection (ESD), have become pivotal in the management of early-stage gastric cancer. Methods We analyzed outcomes from 64 consecutive gastric lesions treated with ESD at our center. The procedure was performed using a conventional approach, involving a circumferential incision followed by submucosal dissection using the DualKnife device. For carcinoma cases, outcomes were evaluated based on the eCura scoring system [ 1 – 3 ] . Results The procedure achieved en bloc resection in 88 % of cases and R0 resection in 72 %. Complications included signifi cant bleeding in 8 % of cases and perforation in 3 %; all complications were managed conservatively, with no procedure-related mortality. Among the 58 neoplastic lesions treated with ESD, 42 cases (72 %) met the criteria for curative resection. Post-ESD surgery was considered for 6 patients (10 %) and performed in 2 cases (3 %). Conclusions In a country with a low incidence of gastric cancer, ESD demonstrates a favorable safety profi le and considerable curative potential. It remains a valuable option for the management of early-stage gastric neoplasms. |
