The Clinic profile and organization structure

Basic specializations of the clinic:

Implantology, transplantology, vitreoretinal surgery, constructive ophthalmosurgery, laser surgery.

Organization structure:

The clinic is divided into Out-patients´ and Ward (In-patients´) parts.  
 
 


Out-patients´ part

This involves two specialized consulting centres:

- for glaucoma

- diabetological


seven diagnotic laboratories:

and four specialized workplaces:

- excimer laser

- out-patients´ department

Consulting centre for Glaucoma
This centre is the only workplace in Moravia that performs both therapy and screening, registration and superconsiliary activities for patients with glaucoma. Complex care is secured in cooperation with
perimetrical and laser (out-patients´ surgery) laboratories and ward department. Due to its top material and technical and staff equipment it is a training centre of the Czech Republic for glaucoma.

Diabetological consulting centre
The consulting centre for ophthalmic complications fo diabetes is specialized in screening, registration and treatment of ophthalmic complications of diabetes whose number has been increasing. The consulting centre works in close cooperation with a
laser laboratory where out-patientsˇ laser therapy for diabetic retinopathy is carried out, and with a ward department where late complications of diabetes are treated surgically (vitreoretinal surgery). The hinterland for surgical treatment covers a large part of Moravia Region. The concept of care of patients with ophthalmic complications of diabetes is a problematic sphere of ophthalmology particularly in early indications for laser peripheral out-patientsˇ or even ward departments, it aims at building up a system of early complex care of patients with diabetic retinopathy.  


Specialized laboratories:

Electrophysiological laboratory
This laboratory belongs to the best technically equipped wrkplaces in the Czech Republic. It performs electroretinographic (ERG) and electrooculographic (EOG) examinations including the examinations of optic evoked potentials (VEP or VER) for the whole region of Moravia and partly even for Bohemia. It is a training centre for electroretinography.

Ultrasound laboratory
This laboratory is a top workplace dealing with ultrasound diagnostics of intraocular and orbital affections. The prior position of the laboratory is geven by the erudition of its workers who are pupils of the founders of ultrasound diagnostics in the whole world (Prof. Vanýsek, Dr. Preisová). For diagnostics there are applied the visualization of A and B for the lesions of the eye and orbit, including standardized echography for differential diagnostics. Besides close cooperation with other laboratories, there is also carried out biometry necessary for the calculation of dioptric value of IOL at the implantation of an artificial intraocular lens after the cararact extraction. Due to its high professional character, it is a training workplace of the Czech Republic.
 

Fluoroangiographic laboratory
Using contrast, it analyses the dynamics of blood flow in the blood-vessels of the retina and choroid. It is the base for making decision on conservative and laser therapy in a number of diseases (diabetes, inflammations, degeneration). It is carried out at ward department of hospitals. Contrast angiographic examination of the iris blood-vessels can be performed only at our workplace within the Moravian Region. The laboratory is a training workplace for Moravia.

Laser laboratory
works with three types of lasers. Continuous thermal Argon and Dye lasers serves for the treatment of diseases of the posterior eye segment (diabetes, degeneration, vascular changes). Dye laser, which enables special interventions within the area of the central region, was the first one in Moravia. Pulse Nd-YAG laser is used for the surgery of lesions in the anterior segment by disruption (secondary cataract, adhesions, fibrous bands). Due to the staff erudition and experience, the laboratory has become a consiliary workplace for the treatment of macular lesions in the hinterland. By introducing
excimer laser therapy for removing refractory defects of the eye, the workplace has been a completely equipped laser centre.

Perimetrical laboratory
It serves for examining visual field by means of statical and kinetic perimetry. Computer perimeter enables quantitative na qualitative analyses of the visual field. The laboratory cooperates closely with
glaucoma and diabetological consulting centres and with electrophysiological laboratory. Special programs enable to examine various diseases of the eye. Several computer perimetres are being used within the region. These are the basic examinations. The hinterland involves large parts of southern and partially northern Moravia.  

Laboratory of corneal topography
A short time ago, corneal topography - computer videokeratoscope enabling quantitative and qualitative analyses of the corneal surface was the only apparatus within the whole Czech Republic. In connection with
excimer laser technique it is spreading gradually, mostly at private excimer-laser workplaces. It is needed in pre-operative and post-operative analyses mainly at transplantation of the cornea and in refractive surgery.

Laboratory of endothelial microscopy
Endothelial microscopy makes possible to judge the state of the corneal endothelium and thus to improve the diagnostics of patients before risk transplantations of the cornea.

Excimer-laser workplace
The workplace of excimer refractive surgery performs the corrections of visual refractory defects. Besides superficial PRK, it also uses, as the only centre within the region, LASIK technique, nowadays the only suitable technique for the correction of higher degrees of myopia, astigmatism and hypermetropia. PTK method is applied here in the therapy for corneal disorders.

Out-patients´ department
In compliance with the world tendency, the Clinic of Ophthalmology, as one of the first workplace in our republic, has started with the out-patients´ performance of some operations - particularly the operations for non-complicated cataract.

Infusotherapy
This workplace is designed for patients with indicated infusion administration of medicaments, but whose conditions do not require hospitalization. Patients can be given adequate treatment even at decreasing the number of beds at the clinic.

Laboratory of computer technique
This laboratory serves for calculating intraocular implants (lens) and their registration. It takes care of database of patients with various disorders requiring statistical processing.
 

In-patients´ part

The ward department serves for the treatment of the most severe diseases of the eye and for post-operative care. It comprises 32 beds (in 1995, there were only 62 beds).
In compliance with the present tendency, the period of hospitalization is shortened, and the exploitation of a bed is increasing. Further restriction of beds is very difficult because of increasing interest in treatment at the Clinic of Ophthalmology, both from the party of private physicians and health-service in-patients´ institutions from the whole region of Moravia and from a part of Bohemia.

The spectrum of in-patients involves especially patients after extraction of complicated cataract, pars plana vitrectomies, perforating keratoplasties and more severe eye traumas. A lot of operations, including non-complicated cataracts, were shifted into the out-patients´ sphere. Hospitalization for diagnostic reasons is exceptional at the Clinic of Ophthalmology. Most examinations necessary for establishing the diagnosis are carried out at the out-patients´ centres.

The basic content of demanding ophthalmic microscopy, both from the view of material, technical and personnel equipment, is the surgery for cataract using all known techniques, incl. phacoemulsification of the lens (high-frequency ultrasound) with implantation of both soft and hard intraocular lens. This clinic, as one of few workplaces, also carries out implantations of thoric lens and it has, as the only workplace in the Czech Republic, a licence for the implantation of intraocular contact lens made by Ameracan firm STAAR. This licence has been given only to a few out-standing ophthalmologic workplace in Europe.

Another sphere of interests is microsurgery of glaucoma, including implantations of special silicone grafts in infaust forms of glaucoma.
