Description Radical cystectomy with pelvic lymphadenectomy represents the standard treatment of muscle-invasive bladder cancer. However, almost half of the patients progress to metastatic disease so surgical treatment alone is not sufficient in a significant proportion of patients. Neoadjuvant chemotherapy to improve these results has been used since the 1980s, and we now have enough evidence for its use in clinical practice. The article provides an overview of importance, purposes, indications, regimen, tolerability and effectivity of neoadjuvant chemotherapy in bladder cancer, including current recommendations.
