Solid bony fusion is a fundamental prerequisite for the treatment of a wide range of spinal conditions. A graft containing both organic and inorganic components is essential to achieve union. An autograft from a healthy individual possesses an ideal balance of biophysical properties, characterised by its capacity for osteoinduction, osteoconduction and osteogenesis. With advancing age and the presence of comorbidities, these ideal properties diminish; moreover, the harvesting process itself carries a risk of morbidity. At the same time, the quantity of graft available decreases. All allogeneic and artificial substitutes combine biophysical properties with the suppression of osteogenesis. Global data point to the potential of cell therapy. The experimental development outlined in this project combines the development of a three-dimensional porous scaffold with suitable physical properties with the application of human mesenchymal stromal cells and endothelial precursors, with the aim of designing and validating, first in vitro and subsequently in vivo in small animals, the production of an ideal living – bio -artificial – osteogenic scaffold with therapeutic potential for the effective induction of stable intervertebral fusion.
