Description Both international and Czech guidelines for the pharmacotherapy of depressive disorders recommend as the first line treatment monotherapy with the newer antidepressants. If the response is not sufficient there are several possibilities - the switch to another antidepressant, augmentation or combinations of antidepressants. In the clinical practice a high percentage of patients is treated with a combination of antidepressants. Reasons for this strategy include the failure of treatment, pharmacoresistance and residual symptoms. From theoretical point of view the combinations of antidepressants influences more ethio-pathogenetic mechanisms, which may play a role in the depressive disorder. A further possibility is the development of antidepressants, which has more mechanisms of actions in one drug - so called multimodal antidepressants.
