Description Giant hiatal hernias have usually sliding component. Due to this findings, we performed fundoplication in all patients with large hiatal hernias, despite they were free of reflux symptoms. This is fundamental part of laparoscopic operation, and may decrease the risk of recurrence of hernia. In our opinion operative treatment is not necessery for small sliding recurrent hiatal hernia in the elderly patients. Results of elective laparoscopic oeprations are very good. Operation treatment should be done in all symptomatic giant hiatal hernias. In symptoms-free patients without serious internal disease we prefere oerative treatment, as prevention of life-threating complications in untreated large hiatal hernias.
