Description Specifities of pain diagnosis in elderly are mentioned in this review article including conditions leading to insufficient pain diagnosis - non emphasizing of pain by older patients, cognitive disorders, influence of social conditions. Pharmacocinetic and pharmacodynamic aspects in old age - decreased biovailability because of decreased resorption, increased fatty tissue, decreased concentration of serum proteins - are discussed and practical clinical consequences are analyzed. History of opioids in medicine is described shortly, their discoveryand usage development up to endorphine discovery, start, development and consequences of opioids misuse and its influence to the current nowadays general view of opioids. As regards undesirable eeffects of opioids, current concepts of the eeffect of treatment with opioids on the influence of opioid treatmentonto gastrointestinal system - nausea and vomiting - are given.
