Description Middle cerebral artery aneurysms belong to most frequent intracranial aneurysms and when rupturing they have the highest rate of intracerebral hernatoma (ICH) because of their anatomical and hemodynamic features. These patients require immediate treatment including operation with uncertain outcome, which is influenced by primary and secondary pathophysiological changes. The significant proportion of patients with MCA aneurysm and ICH may achieve favorable outcome, however final outcome seems to be influenced by hematoma size and the presence of vasospasms. We did not find the difference in outcome between right and left hemisphere. The size of hematoma and edema influence the operative strategy.
