Description Introduction: Ovarian carcinoma occurs in all age groups of women. In 2008, according to NCR (National Cancer Registry) in the Czech Republic 1161 cases of newly diagnosed ovarian cancer at an incidence of 21.9 / 100000 women. More than 70% of cases are still diagnosed in advanced stages III - IV. Cancer screening does not exist and all efforts to implement it (CA 125, ultrasound, a combination of both) have not yet proved to be ineffective. Patients in clinical stage I. achieve 5-year survival in 80-90% of patients II. clinical stage survive 5 years of 60% in the third. clinical stage in 23% and IV. clinical stage less than 10%. Objective: The aim is to retrospectively evaluate the sensitivity of simple ultrasound criteria in the hands of an experienced sonographer sensitivity of tumor marker CA 125 and each of the two modalities in the preoperative staging subsequently detected in ovarian cancer. Methods: For all patients with findings suspicious adnexal masses in the countryside, which were treated at the Department of Gynaecology and Obstetrics, University Hospital Brno in the years 2010 - 2011, was conducted expert Oncogynecological ultrasound and CA 125 were collected Subsequently retrospectively evaluated the sensitivity of preoperative staging investigations in patients with histologically confirmed ovarian cancer. Results: In the years 2010 - 2011 were diagnosed in our department a total of 87 ovarian cancers. The reference level for positivity CA 125 is determined in our laboratory for 35 kU / L. Of the total 87 patients with ovarian cancer were ultrasound in 78 cases, with the conclusion of cancer suspected to be malignant, in 9 cases the ultrasound finding nesuspektní cancer, which represents the sensitivity of ultrasound to detect ovarian cancer 90%. Of the total 87 patients with ovarian cancer was taking CA 125 assessed as positive in 56 cases, 31 cases negative, which represents the sensitivity of CA 125 tests for the detection of ovarian cancer 64%. The difference in sensitivity between the expert preoperative ultrasound examination and collection of CA 125 in detecting ovarian cancer in our study is 26%. Conclusions: Expert preoperative ultrasound (U.S.) examination accurately divide the pathology of benign, malignant and uncertain nature. Adding take CA 125 to any of these groups does not increase the detection of ovarian cancer. CA 125 is not superior to expert ultrasound examinations in determining the nature of the lesion. The sensitivity of CA 125 is lower than the sensitivity of ultrasound expert, clearly sensitive to ultrasound diagnostics appear in the earlier stages of the disease.
