Description Ulipristal acetate (UPA) is a selective progesterone receptor modulator with a tissue-inhibitory effect, which we use for conservative preoperative pharmacological treatment of moderate to severe symptoms of uterine fibroids disease in adult women of reproductive age. UPA stops excessive bleeding quickly (during one week), normalizes menstrual bleeding and induces amenorrhea in the majority of patients with uterine leiomyomas. It significantly reduces fibroid size and improves quality of life. Pharmacotherapy should be individual. For symptomatic women of reproductive age planning a pregnancy, it is possible (depending on the extent and location of the fibroids) to use UPA to prepare these patients for surgery by reducing fibroid size and subsequently indicating the patients for myomectomy. UPA can also be used post-operatively to reduce residual findings and in the treatment of inoperable findings (multiple fibroids).
