Description Prevention is the most important strategy in the fight against one of the most serious medical conditions worldwide – cerebrovascular accidents. This article gives an overview of the three most important innovations in the field of prevention of ischemic cerebrovascular accidents, made in years 2011 and 2012. These three innovations are concerned with the two primary high-risk causes of ischemic stroke, which is intracranial atherosclerotic stenosis and atrial fibrillation. The third possible cause, which is largely theoretical and less common is patent foramen ovale. The importance of patent foramen ovale stems from the fact that patent foramen ovale is a possible cause of ischemic stroke in young patients without risk factors and from availability of an intervention method for its treatment. In this article, we show that the new anticoagulants, such as dabigatran, rivaroxaban and apixaban represent a breakthrough in prevention of ischemic stroke due to atrial fibrillation and that patients with ischemic stroke have the same benefit from preventive therapy as patients without ischemic stroke. Conversely, although catheter interventions such as angioplasty/stenting of intracranial stenosis or percutaneous patent foramen ovale closures are an elegant technical solution, they do not at this time provide sufficient benefit to patients compared to conservative treatment.
