Description Medullary carcinoma represents about 8% in thyroid malignancies. It occurs sporadically, familiarly or it may be a part of the MEN2 syndrome. Sonography with fine needle aspiration biopsy (FNAB), biochemical tests (calcitonin) and scintigraphy (131,123) I-MIBG serve to diagnosis. Mutation of RET proto-oncogene is typical for familial forms. Early thyroidectomy for RET proto-oncogene mutation carriers helps to prevent disease. Biological treatment (cabozantinib) is indicated for locally advanced unresectable and metastatic medullary carcinoma. Therapy of radionuclide 131I-MIBG is effective in patients with metastases with positive scintigraphy 131I-MIBG.
