Description The aim of our communication is to present our methodology we use in the treatment of idiopathic macular hole and which are in terms of achievements seems optimal. We provide standard 3port PPV, which includes complete removal of vitreous, posterior hyaloid membrane elevation, and its removal to the periphery. If present some degree of cataract, PPV is a combined with lens operation. Internal limiting membrane (ILM) is removed using a trypan blue dye (Membrane blue, DORC). At the end of the operation we provided for water-gas exchange mixture of 15% C3F8 (perflouropropan). Patients are instructed to follow a period of 1 week facedown positioning. Surgery is indicated for patients when is diagnosed with macular hole stage 2, 3 and 4 according to Gass. Stage 1a and 1b are not operated for a high percentage of spontaneous regression. PPV supplemented by internal limiting membrane peeling and expansive gas tamponade is a safe method for the treatment of idiopathic macular holes. If is before surgery diagnosed cataract, PPV is combined with lens surgery.
