Description Early initiation of hypertension treatment and rapid achievement of target blood pressure improve patients' clinical outcomes and significantly reduce the overall cardiovascular risk. At present, the strategy based on the initial combination seems to be the most appropriate treatment using a fixed combination that will achieve the goal as quickly and safely as possible and minimize the risk of non-adherence patients and clinical inertia of physicians. A fixed combination of a renin-angiotensin system blocker with a calcium blocker is recommended blocker or diuretic. At least 30% of hypertensives require a triple combination and even this optimally fixed - eg perindopril + amlodipine + indapamide. Classical titration of treatment from monotherapy is suitable only for mild hypertension or for old and fragile ones hypertensive patients, because otherwise it delays the achievement of good blood pressure control and increases the risk of non-adherence of the patient or therapeutic inertia of the doctor.
