| Description | The placement of working ports, auxiliary thoracotomy, patient position, position of robotic cart and especialy technic of blood vessels ligation are discused in this work.The authors compare own first experiences in robotic asissted lobectomy with the other surgeons experinces reported on site Medline.In our department were performed 3 robotic assisted lobecomy of two woman and one man (45,52 and 57 years old.) The left lower lobectomy was done twice and once at the right side. The blood vessels were ligated mechanicaly using the robot. The mediastinal lymphadenecomy was always performed following lung resection. The auxiliary minithoracotomy of lenth 5cm was placed posterolateraly. There weren t any complications neither peroperatively nor postoperatively.The robotic assisted lobectomy is safe method of pulmonary resection for early stage(Ia,Ib) of bronchogenic lung cancer. The choice of working port depends especialy on tumor specimen location and is very important for free movement of robotic arms and camera to exact preparation. Right choice of auxiliary minitoracotomy placement plays important role not only to help for lung fissure dividing by stapler, but especialy for posibility of emergent conversion to open operation. |
