The Treatment Results of Patients after Aneurysmal Sah – the Comparison of the Groups under 70 and over 70 Years of Age

Popis Introduction: The aim of this work was to compare the treatment results of patients after aneurysmal SAH with regards to age. Material and Method: We enroled 342 patients treated in years 2006-2012. 45 pacients were over 70 years and 297 patients were under 70 years of age. Their status in admission (HH), SAH severity (Fischer), methods to treat the aneurysm and the treatment result (GOS) were evaluated. For statistics Fischers exact test an Chisquare test were used. Results: There was no diference in clinical status (HH) of the groups, p=0,1804. There was a diference in the SAH severity, p=0,0067, more severe in senior group. The conservative treatment in the group over 70 was in 29%, under 70 in 13%, (p=0,0182). In patients with intervention endovascular treatment in the senior group was used in 31%, surgery in 69%, under 70 there was 27% of endovascular treatment and 73% of surgery, which is not different (p = 0,6747). The overall result were worse in the senior group, p=0,0192. Mortality (GOS1) in senior group was 44%, in younger 25% (p = 0, 012). The good result (GOS 4+5) in senior group was 38%, in younger 56% (p = 0, 0169). Conclusions: Patients over 70 years of age have a trend towards a more severe SAH clinically and significantly on CT. The higher percentage of these patients is treated conservatively. If it comes to intervention, surprisingly there was no difference between the groups in the method used. The overall treatment results in senior group were significantly worse.
